WELL AGED PROGRAM *

Objective

Serve those persons who are 60 years or older and who are experiencing, or at risk of experiencing, social isolation through offering Community Support and Information, advice and referral services.

Community Support

We promote greater public awareness of social issues and enhance individual and community group capacity.

We do this by:

Developing opportunities for older people to participate in programs/events/activities/projects designed to develop their skills and support them in reaching their full potential.

Including older people in program/project development including consultation,. design, delivery and evaluation.

Providing opportunities for older people to express their ideas, raise issues of concern to them, and act on issues which affect their lives leading them to become engaged and resilient.

Developing opportunities for the wider community to listen to older people and acknowledge and celebrate their positive contributions made by their efforts and achievements.

Supporting older people to positively connect with their families, friends and social networks.

Delivering programs/events/activities/projects in a culturally appropriate manner

Information, Advice and Referral

We provide services that assist service users and community members to make informed decisions about, or be connected to, the services and support that they need. This service is an important aspect of both prevention and early intervention which may involve preliminary needs identification for the purpose of identifying appropriate service and support options.

This service will ensure to provide older people, their family and friends, grandparents and carers with access to information on topics such as:

* financial and legal matters

* social activities

* household assistance

* retirement accommodation

* health

* education

* transport
