Support At Home (SAH) Program

The Support at Home Program is

A single, streamlined system that gives older Australians access to the services, equipment, and home modifications they need to live independently, safely, and comfortably at home for longer.

It brings together:

Ongoing in‑home care (cleaning, meals, personal care, nursing, transport, social support)

Short‑term care pathways (Restorative Care, Assistive Technology & Home Modifications, End‑of‑Life support)

A new funding model with 8 ongoing classifications and 3 short‑term classifications

Stronger safety, quality, and rights protections under the new Aged Care Act

How it works

1. Assessment

You start with a My Aged Care assessment.
You’re assigned a classification based on your needs, which determines your quarterly budget.

2. Your services

You receive a list of approved services you can use your funding for, including:

  • Everyday living support (cleaning, meals, shopping)

  • Independence support (showering, medication prompts, transport)

  • Clinical care (nursing, physio, OT, podiatry)

  • Cultural and community connection supports

3. Your choice of management

You can choose:

  • Provider‑managed

  • Self‑managed

  • Partially self‑managed

Self‑management gives you more control over workers, schedules, and spending — with a Coordination Service still ensuring safety, compliance, and disaster readiness.

Short‑term pathways

These sit alongside your ongoing care and don’t reduce your main budget:

  • Restorative Care Pathway – short-term allied health to regain function

  • Assistive Technology & Home Modifications (AT‑HM) – upfront funding for equipment and home changes

  • End‑of‑Life Pathway – extra support to stay at home in your final months
